I can't see how that can be guaranteed. The really, really popular places, such as Hero and V-Club, probably yes, because they tend to attract Asian customers. Generally speaking, Asian customers like the bigger boys, and teenagers don't have the physical build to be that size (again, speaking generally).
Places that are aimed at western customers, e.g. Bonny, I'd be less sure, because a significant number (not all, I stress) of westerners who vacation in Thailand go there precisely because of a stereotype that they are chasing - that of younger, smaller-built boys. So establishments that cater to this clientele could veer pretty close to the borderline, maybe even cross it.
There is/was one place, which I shall not name for obvious reasons, which had two different line-ups. Any customer who walks in would be given the first line-up consisting of guys in their twenties. The mamasan watches the customer's eyes to suss out his interest. If the eyes tend toward the youngest and smallest of that line-up, and if he does really look like a tourist (from ethnicity or accent), then the mamasan may mention that there is another possible line-up inside another room. No prizes for guessing what that line-up consists of.
