Since pong himself said he hadn't been into a gogo bar in 5 - 6 years, I think maybe I'm better placed to answer your question (also, from his description, he must have seen the short, midnight, version of the 10:30 pm show I saw).
Generally, the bars do have stand-by performers. Firstly, because the mgt do plan for the eventuality and secondly, the boys doing the act earn quite a lot of tips from going around the room, so boys do volunteer to do it.
If there are no stand-by performers, or if the boy, despite having a prospective customer, still wants to do the show, he and the mamasan will try to negotiate with the customer to wait till after the show before the boy is taken off. (This happened to me recently at Future Boy, though the boy in question was doing a suckfest act, not a coupling show)
Alternatively, as a last resort should the boy be taken off and there is no replacement, that item is left out of the show.
As for your first question, my guess is that Pong was surprised to see two masculine guys doing the coupling act. In the old days - 5 or 6 years ago - it was de rigour for the bottom to be a fem.
